Upon acceptance of enrollment in our school, you'll need to complete the following:

Original Birth Certificate or certified copy

Completed Ogden School Enrollment Form (if possible)

Parent/Guardian proof of residency (2 Forms Required) Utility Bills are preferred

Consent Form and Release

Recent Standardized Test Scores (e.g. ISAT)

Recent Benchmark Assessment Scores (e.g. NWEA)

Recent Grade Report (8th Grade)

Supply Fee ($ 250- Cash or Money Order)
